Proof of Immunization

New York State Public Health Law 2165 requires that all students born on or after January 1, 1957 who are enrolling for six or more college credits provide proof of immunity to Measles, Mumps and Rubella (MMR). Proof of immunity must be provided to the NYU Health Center by submitting (1) a Student Immunization Form or (2) a letter or other documentation from a doctor or health care provider. Please send all forms to NYU Health Center, 726 Broadway, 3rd and 4th floors, New York, NY 10003.

For more information, contact the NYU Health Center.  Students who only take online classes and who are not ever physically present on campus may seek a waiver.
